Parents, students, and/or legal guardians may request a copy of the student’s special education records via phone or by completing this form. You will be notified as soon as the records are available for pickup at the main Macon-Piatt Special Education District office, 620 E. Garfield Avenue, Decatur, IL 62526. Please allow up to 10 business days for your request to be processed.

Important: A valid photo ID will be required to pick up copies of records.
